Elevated HR after training (HR increases over MAX HR!)

Hi Hubbers

 

Last weekend at Around the Pot something strange happened for the first time. Due to some mechanicals in the first 20km we chased back to the main waterpoint at 100km to try and make up some time. The last 20km I did a lot of the work in front with a head wind, and didn't drink and eat enough as I usually do (my mistake). As soon as we stopped at the 100km waterpoint and I climbed off my bike, I felt something strange happening inside my chest. 

 

Personal info:

- 32 years old, fit

- Training consistently 10 hours p/week (2 x HIIT sessions p/week for the last 2 years) with recovery in between (training balance)

- Avg 6 rides p/week

- Max HR 197

- Resting HR 49

- 165cm, 64kg

- Don't take any medication (haven't been sick in probably +9 months)

- Logged 7274 km to date (2018)

 

Back at the waterpoint I started feeling light headed. As soon as I looked at my HR monitor, my HR was around 192 - 201. It didn't want to come down. Chest felt weird as like the Heart was in a panic mode. 10min later we went over the river and on the other side as we went out with a easy group, the HR went up to 209. Told my partner something is definitely wrong and we stopped, standing still for around 5min next to our bikes. In that 5min the HR didn't come down lower than 182. We pulled the plug right there. Went back to the waterpoint to our support and in total it took about 20min for the HR to calm down and return to normal.

 

Last week Friday I saw a Cardiologist, Dr Corbett in Panorama. They did a ECG and sonar of the HR. The conclusion: Heart is perfect with no damage to muscles/chambers. The effort might have just been to much with dehydration etc playing a role.

Did some easy HIIT classes again last week which were normal, and HR recovering normal in between sets.

 

This morning went for a ride outside (in the wind) and one section pushed HR between 170 -181 for around 15min consistently. As soon as I stopped in Stellenbosch it happened again, HR up for about 10min before calming down.

 

Has this happened with anybody else?

Any idea what would cause this?

While you exercise you have muscle contraction functioning as a pump forcing blood back up to the heart.

 

As soon as you stop you have a drop in venous blood return (no muscle pump), this causes a decrease in pre-load on the heart causing a decrease in contractility and therefore stroke volume. To try and maintain blood pressure and overall cardiac output, the body will elevate heart rate.

 

Best thing is not to stop abruptly, but rather to continue moving to keep some muscle pump going. Of you ly down it will also help, as gravity will aid venous blood return to the heart.

 

Dehydration obviously doesn’t help

so you have a new max hr now.

fwiw my max is 205, was worried about it, doc ran the tests, all normal.

This exact thing happened to me on the weekend (except I was just riding easy up chappies and my HR shot up for no reason).

 

Iam 30 years old, female, 168cm and 58kg and my training programme and 2018 stats very similar to yours.

 

My dad is a cardiologist. He said this is extremely common in young athletes and is most likely just an SVT (supra ventricular tachycardia) which occurs from time to time. Dehydration also definitely plays a role.

 

He did an echo and resting ECG on me this week and everything looks normal. I am going back for a stress test later this week to just check what my heart does under exercise, but otherwise all is good and I’ve been told to carry on as normal but just keep tabs on it if it happens again. Slowmag apparently also helps.

 

You did the right thing going to see someone about it - Clive is a good guy. Is he said you’re good to go then I would listen to him. Just keep your hydration levels under check and keep tabs on whether it happens again.

Both times you were riding into the wind, maybe you allergic to head winds?

I also normally have a high HR but ive had something similar happen, its only been in races where the pace goes frantic from the word go, so its basically full gas from the start, I can last like 5mins and then all of a sudden, also get like a kinda numb feeling coming over me and HR goes to like 220, as soon as I step off the gas and stop pedalling and chill for about 30sec it drops straight back to "normal" and I can continue racing as almost if nothing happened. Difficult to reproduce and also nothing wrong with my heart after tests

Any idea what would cause this?

1 - cut back your exercise immediately till this is investigated understood properly.

 

2 - make an appointment with a cardiologist for a proper exam and stress test. Take your garmin traces with you.

 

If it happens at rest - get someone to take you to an ER immediately.

 

If it happens when driving - pull over immediately.

Thanks, will take it easy now for a few weeks.

 

Both incidents where HR shot up were similiar. Doing a 15-20min consistent pace with HR between 170 - 181, feeling comfortable with no warning signs. Both cases now were as soon as I stopped the HR dropped and then suddenly went up and feels like it goes into a hyperactive mode (this morning 150 - 175 for 10min sitting still..)

 

Will contact Cardiologist again tomorrow.

If it happens at rest - get someone to take you to an ER immediately.

If it happens when driving - pull over immediately.

My thoughts exactly - but it might be hard to find given he is quite young still.

If you going back to cardiologist try go to someone that is a specialist in arythmias.

Dr Faizel Lorgat in Cape Town is very good.
