Since the

start of the Tour the riders have spent more than 100 hours in the saddle.

Their perineal zone, on which they sit, is therefore subject to numerous

problems. To avoid the problems of overheating, infection of hair follicles, and

even more serious medical issues such as the famous ?third testicle?, riders

have to take great care of this area.

 

 

Gerard

Guillaume, doctor with FdJ, insists on the key importance of preventing

lesions. ?Today, unfortunately, we sacrifice comfort for performance. Saddles

have become lighter and lighter, therefore slimmer.

 

 

 

But 40% of

the weight of a cyclist rests on the saddle, and the perineal zone is therefore

subject to a great deal of rubbing and compression so that its surface area, on

which it rests, is thin and weak.

 

 

 

We need to

use wider saddles, without a gap in the centre ( because with them the pressure

is greater on the sides, where the lesions are worst) , and without any

stitching. The other way to avoid problems is to use a less rigid bike, a

saddle specifically chosen to fit, and to take really good care of racing

shorts.

 

The shorts

should be washed by hand ( preferably) or by machine with non aggressive

washing powders. And above all, they should not be dried on a washing line!".

 

 

 

 

 

Also you

can apply a cream before putting on your shorts. ?the best are those with a

Zinc oxide base, available at chemists? continues Dr Guillaume, ? But you can

also use (baby) talcum powder?.

 

 

Nicolas

Bonenfant, a manufacturer of racing shorts, insists on another point. ?Your

shorts must be like a true second skin, and not move at all. Choosing exactly

the correct size can avoid so many problems?

i got septcimia  from a saddle sore, landed in emergency surgery and was  in isolation for a week and was of the bike for 8 weeks,take care use all the creams etc and get into dry kit asap,and use alcohol to wipe if you cant get to a shower straight away

I've used the Assos cream and it works quite well....

 

Third testicle - doesn't everyone have one ?

THE FAMOUS THIRD TESTICLE:

 

 

 

The cytologic and histologic findings from two cases of perineal nodular indurations observed in two

 

cyclists are reported. These lesions, also referred to as "accessory testicles" or "third testicle"

 

or "ischial hygromas" of cyclists, consist of a localized aseptic area of necrosis with pseudocyst

 

formation involving connective tissue in the superficial fascia of the perineum. These histologic

 

findings, which were seen in the subsequent surgical specimens in these two cases, were reflected in

 

the fine needle aspiration findings. The aspirates contained few cellular elements, mainly a few

 

vacuolated histiocytes, against a background of fibrinous material. These indurations, which develop

 

as a result of repeated, chronic microtrauma to the perineum impressed by the vibration of the

 

saddle of the bicycle, constitute an authentic handicap for the professional cyclist and are a

 

contraindication to cycling for amateur cyclists.
