PRN Plates

Just a quick question do somebody who might have changed their plates from one car to another.

 

I sold my one car which I had these personalised plates on and would like to transfer it to my "new" car.

 

What is the process and which forms are needed? I'm quickly going to fetch the change of ownership papers this afternoon and if other papers are needed also collecting these as well.
