Use old-fashioned toe clips? You don't need to ride with the straps tightened, although that means you can't really get any power on the up-stroke (Note: personal opinion is that one doesn't get much anyway). But the toe clip does at least keep your foot relatively positioned on the pedal. You can get cheapy ones at Sportsman's Warehouse, though you have to use those with beartrap MTB pedals, or try find a more elegant pair like mine (proudly puffs chest).

I had the same problem after an accident, I've lost the range of motion in my ankle, after a year of riding with toe clips I took the plunge and got cleats, couldn't unclip with regular shimano cleats so did some research and decided to try shimano multi release cleats they've worked a charm(they're almost impossible to come by in SA so order online)

There is some concern that the multi release could be a hazard because you might uncleat unintentionnaly hasn't happened to me but then again it is probably applies to super strong agressive riders?
