Perhaps touching on the hub axle and not tightening properly - I had to swap out my hope hub axle end cap because it was binding before the cassette was fully tight.

I've come across some crappy freehubs.  As they can be made by any hub manufacturer the quality can vary a lot.  On one the casting was so bad that I had to machine the shoulder just in front of the threads to get any thread contact at all.  On another the splines did not engage well and the cassette bit into the free hub body.  This created a path of least resistance so the cassette never lined up on subsequent fittings.  I had to use some creative marking and use of a chain whip to get it fitted right.

 

As pointed out you need a torque wrench to fit this cassette - and a bit of lube.  

 

Not the greatest design.  Reminds me of the Octolink BB axle which you also fitted blind.  Depending on what seal your freehub has you can put two bolts in a vice to hold the cassette while you tighten the freehub.  This lets you see what is happening.  Then fit the whole thing to the axle.

It happened again. I did use a torque wrench and after about 8 - 10 rides it has now come loose again.

 

Its a Roval Wheelset with DT Swiss Rear Hub and DT Swiss XD Driver.
