There is no correct way.  Like with bike sizing (C-to-T, C-to-C) they vary by manufacturer.  The best way is to define your required size no matter which method is used.  If you are looking at C-C measured bars then measure your shoulders from the middle of the joint on the left to the same point on your right.  If you are looking at outer to outer then measure your should width in the same way.  Note that bar width is one of those measurements that is not exact science.  Some guys prefer smaller than "ideal" bars and some prefer larger.  Personally, I have 47cm wide shoulders (outer to outer) and my bars are 45 (C-to-C) which equates to close to my shoulder width.

From where to where is of no importance.  You decide you want outer to outer (easier as to fix the centre is difficult) then you take a tap measure and measure the damn bars.  Obviously bars are about 2cm in diameter so subtract that from the o-o to get the c-c.  The normal sizing is to take your suit size (36 cm or 40 cm) add a bit say 4 cm and then experiment.  I have a 36/8cm suit size and ride a 40cm bar.
