It's the difference between the pedals. Lets say you turn your cranks so that both are at the bottom. You measure it then. I like a wider distance as it feels more natural.

I have been having a hassle with my lower back and hips and one of the guys i ride with told me to check Q factor.

 

On my mtb is much wider than my road bike and it's when i have ridden my mtb that i am having hassles.

Is there a way to get it narrower other than buying new cranks?

See if you can move your cleats towards the outside of your shoes. This will bring your feet in. I'm the other way. I like the MTB and have the hassles with the road bike. I think you get shorter or longer axles for Eggbeaters to help with this problem aswell.

What is the Q-factor in cranks and what impact does it have?

I read Sheldon Browns explanation but don't really understand.

Q-Factor affects those with a wide (__.__) and narrow (_._)

you ride with your feet further apart or closer together..
