I do have this issue with my road bike brakes. I do not ride the road bike that frequently more on the MTB and then I for get about this issue.

 

Whenever the brakes was applied it seems that the brakes tilted more to one side and rubs against the one side of the wheel. I center the brakes and after the first application the problem is back. I tried to tighten the brake calipers slightly more but afraid that it will be to much and thus breaking the carbon frame. The brake block wear is about even.

Brakes is Sora.

 

Should I remove clean and re-fit?

There is a nut that clamps the brake set against the frame. I think it is a size 13 spanner. You need to center the brakes with the spanner on that nut, then tighten it from the back with the alan key. Front and back brakes are similar.

I had this issue with a set of 2nd hand Tiagra brakes on my dad's bike. As you mentioned, centering the brakes does nothing to resolve the problem.

 

I sorted this out by removing the calipers, and servicing them fully, i.e. dismantling them completely, cleaning all parts and pivots with engine cleaner - there was a fair amount of muck build up and rust, more cleaning, drying all parts and grease/oiling them where necessary. After that I refitted them to the bike, centered and they are working as good as new again.

 

So yes, remove, clean properly and re-fit.

Check that the lock washer is still in place - between the nut and the frame.  The alan key nut should be tightened to 6 nm which is "firm" with a multi tool. Just tight enough that you can't rock the calliper by hand.
