chain slip on heavy gear, Solution?

Worn cassette or the amount of chain wrap on the cassette. If  the B screw is too far in and the derailleur thus pushed back too far away from the cassette.

On what type of frame? Carbon/Steel/Alu?

 

If there is enough flex in the frame (e.g. steel frame), you might find the chain will slip under big power on the small cog because of a slight twist in the rear triangle.

 

Iv'e seen this happen once on my steel MTB frame when a 100kg+ chap climbed on the bike and stomped on the pedals in the small cog - chain slipped. I could not repeat this with my weenie power output... just a thought.
