just my 2 cents. Had the same problem. with the sore feet I got new shoes that fit right, get the fit right. With the toes going numb, this could also be that you are cutting off the blood supply, I stopped wearing undies with my cycling shorts and changed my seat. the thinner seat (width wise) help alot.

No need to modify the cleats, just the shoe sole.

They are Time mtb cleats which attach to the shoe with 2 screws, the shoe sole has 2 parallel slots which the cleats screw onto, you slide them back or forwards in the slots and then tighten them down when they are in the desired position. I lengthened the slots +/- 15mm (towards the heel of the shoe) and slide thecleats all the way back in the now lengthened slots….

"hotspots" shouldn't make toes go numb. Include Morton's Neuroma in your diff. diagnosis. Similar to carpal tunnel in mechanism. Cycling shoes tend to compress the metatarsals. Cleat backwards is always a great idea unless your are Pro and really need each mm of leverage.
