I have gone the patch from the inside route. I however did not use a normal bike patch, I got a "mushroom" patch from SupaQuick, cost me about R5.00 if memory serves.

I cleaned the tyre are properly, sanded it lightly to ensure even adhesion, glue the crap out of it (including the hole in the tyre), left to dry a bit as per usual patch and stuck the mushroom through and on. After about 15 mins I just cut the excess piece sticking out the tyre flush, left for a day. Added sealant and it worked a charm! Tyre was at about 70% lifespan left when I patched it and I had rode it until about 10% tread left with no issues before I replaced the tyre.

I suggested this to a customer earlier today. More like R50 though...

Also if you leave the really long thorns in they will pik away at the rim tape every time the part of the tyre with the thorn in gets squashed at the bottom of the rotation of the wheel.
