Yup. Seen both.

 

Right up there with that mark that waterpump pliers leave on "things I didn't have the tool for".

 

Bottom Brackets being the favorite.

That gap seems excessive will cause a lot of gunk going in there, Is that the crown race that came with the headset, or the one from the previous headset ?

I think they stacked a new crown race on top of an old one. Seen a local (well regarded) LBS do this when fitting a 2nd hand fork for a friend. The bike ended up with me a few rides later when the headset started rattling. The problem with stacking crown races is the underside of the top one is not supported across its full width and it soon deforms when the rider's weight acts on it.....then the headset goes loose.....

 

If not stacked crown races then its the wrong bearing for that crown race.
