Biggest mistake some people make is to over tightening a bolt/nut/screw.

 

Take car wheelnuts as a example... the spanner is a certain size for a reason - to limit the amount of torque that can be applied when tightening the wheel bolts/nuts. Problem is, that some people stand on the spanner to ensure the nuts are tight, rather than use the (simple) design of the spanner. It is a certain length for a reason... more leverage = more torque.
