If you ride hecticly bumpy terrain, and the chainline is slightly off, you may find your ride stopping more often than not. ( obviously mtb) Road, you ma get away with it for longer. If you do run a tensioner and are worried about aesthetics you can try a BB mounted 1x guide. Can work well but adjustment can be tricky.
