Maybe this is a bit "way out", but having his name and address couldn't the card be posted back to him? Not risk-free, I grant you but possible?

 

Sorry if it's a stupid idea & I'm missing something really obvious.

guy in charge of maintenance... Notice how  he manages to get the lines

to cross (senior  management)
