Screw sets the preload tension on the piece of spring steel that controls the ease of movement of the Rd cage, that's on Shimano Rd not sure about SRAM but probably similar. Unless that screw is coming loose it's a set once and forget thing, maybe once a year or every 6 months or so if you ride a shed load of distance on very harsh terrain with the RD, I suppose in that case the friction over time may wear things a little bit and it'll loosen up/need more tension.

 

Easy to check if the tension is within spec, just test the force needed to push the RD cage forward, also if chain slap increases on routes you know well when there was previously none or little - but then again that just may be because you are charging harder than before ;)
