I need some advice. After producing a big effort my bike started devoloping a loud creak which got louder and louder. It was clear that the BB had failed. However inspecting the bike at home I realised that the BB started pulling out of the threads of the frame on the top. Is the frame toast? Has anyone ever experienced this?

That cup will need to be removed and the BB threads inspected by a qualified bike mechanic with a thread gauge.

If the threads are damaged it may be salvageable through re-cutting the threads with the appropriate thread tap.

Not sure where you're based to direct you to a reputable bike mechanic

If the thread is beyond recutting you can get threadless "repair BB's".  I have used the Velo-orange one, which is great as the bearings are sealed and replaceable. Not sure if you can get anything other than square taper 'though.
