Try the people who do rubberizing on bakkies etc. They should have the tool. You should be able to buy diff sizes rivnuts from any bolt shop.

Keep trying till you find the right shoe, a too large shoe is just as irritating than a too small shoe. I took my shoe inners out to allow a little bit more space. Try North Wave Iguana if you can still find them...
