The RD must be in the 9th gear (smallest) as well ......

OK - firstly I would suggest undo the cable at the RD then hold the cable and shift the shifter and feel if the cable moves .... If not then the problem lies at the shifter ...... You must thread the cable through when the shifter is in 9th gear!
