Where to get steerer tube extenders?

Plenty of seemingly happy people mounting that Velobike one to mtbs and the like. 
The extended plug with half of the stem on the original steerer makes me feel okay-ish with the concept.

I'd take the fork to an engineering shop and have them mill a custom alum plug for the steerer and then just bond/glue it into the top of the carbon steerer tube. With modern bonding agents it will never come loose, especially if you insert it around 10cm. If you've ever tried getting an stuck alum seatpost out of a carbon frame you will understand why I say this and that's without any bonding agent present.

This . Cut the steerer halfway and insert and glue in die middle
