Spez shoes. I had the same problem and since changing to spez shoes never looked back.

 

Also ensure that your cleat is position correctly.

Do you have wide feet ?

 

Shimano have a wide option, R171 / RC7 or RC9.

 

Using the R171 for 2 years now, no issues. Will be upgrading to RC9 soon.

I have not bought new shoes in years and still ride a pair of Look road shoes . What i did from day one is to put a good inner sole in . This helps with underfoot padding and also stretching the upper portion of the shoe . I f you have a wide foot  then wet the shoes with a hose before you go on a few short training rides . This helps stretch them and allows the shoe to mold around your shape . ( sounds like an old wives remedy  but riding in the rain has the same effect )

I can also recommend a brand of shoe... But it's different from all the above...

 

Everything listed is pretty good advice/options in any case.

 

Perhaps look at a custom mouldable insole as a start.

 

Cleat positioning can be a cause too.
