After one and a half years of struggling with ITBS and having tried everything, except surgery, I am going under the knife on Friday. It has just gotten to a point where I cannot run more than 3km and bearly cycle for an hour. I am a little worried about surgery, for obvious reasons. Are there any guys out there who has had this surgery and what has the results being. Also how slowly should I start training again given that I need at least four months to be ready for Ironman 70.3 in January.

Provided you have a reputable surgeon I wouldn't worry.

I've had both legs done on separate occasion and am 100% happy with both outcomes. The most important thing is to resume (mild) training as soon as you can (Usually 1 to 2 weeks after surgery).

My left leg was done first and I got back on the bike within 12 days, started with a 3km ride. Every 2nd day after that I managed to double the distance. Remember to keep resistance minimal, your body will tell you if you are pushing too hard.

My right leg was done a couple months ago and I was on the bike within a week, my training hasn't been as frequent as when I had my left leg done and this has increased the recovery time, you must use the leg/s within 2 weeks and try keep at it, at least twice a week to speed up the recovery :)

Next to laser eye surgery this has been the best money I (My medical) have spent :) Many people are against the knife but if you battle with physio and exercises for longer than 2 months with no results it's the only option, and a pretty good one at that as far as I'm concerned!

my mate had both his done, was back on the bike in 3 weeks, back running a slow 5km in 4 weeks. He did a little swimming in the 2 weeks after surgery. The procedure was done in April this year.

He battled for 6 months with every therapy, I could think of and he is now prepping for some racing.

Thanks for the feedback guys. I have also heard only positive comments about the results. Sounds like January 2013 IM 70.3 is then not an unrealistic goal.

So another question, how bad should it be before you do this. I have no issues with any sports except running. I can't run more than about 3km. Sometimes less. Yet I can ride marathons swim row etc. Also adding to the other question. How can you be sure its ITB?

After battling for 18 months I had mine done in Dec last year and was back on the bike riding 90+km after a month. Best thing I ever did as none of the other treatments helped.

Yes a small cut on the side of the knee and that releases the itb and it stretches out a little. The pain on the side of the knee you usually get after running or cycling will never return

I am back from my surgery. In at 8 this morning and out of hospital @11. Dr says everything went well, now I need to rest and can start cycling after about 10 days. Did any of you guys go for physio after the surgery? Dr says it might be a good idea after a week or so.
