i'm having a slow morning: what has the crank arm got to do with bike being on a rack? Why was it loose to begin with?

Was thinking the same thing... must have ended the ride on one pedal!!!

Well it fell off during the ride and did the best I could to ghetto fit it back on and tighten the bolts and it stayed on until I got back to the carpark.put the bike on the rack and it fell off the bike again whilst driving out

Ghetto fit?? You only need an allen key to tighten it properly?

IIRC, that's (crank cap) only for BB preload and not to hold the crank to the spindle.. The two allen bolts are required to tighten the crank to the crank spindle. musn't forget the spacer to fill the slot in the crank. its that spacer that prevents the cranks from falling off if you forget to tighten the allen screws.

you are right, but the arm shouldn't just fall off if the cap is fitted.

That cap only needs to be hand tighten, any tighter and it may lock.

Not many people carry a ten mil Allen key, found that out the hard way at sani2c last year, couldn't even get one at the water points.
