So after 13500kms of riding in under 2 years, I get on my bike this morning, cleat up my right foot, lose my balance  and fall over!!!  Hasn't happened in a while but seriously, should this happen at all?

Not nice, but it happens if the cleat get stuck in the pedal. I've seen some guys struggle at the 94.7 after they had to walk a bit of sand with their road bike shoes, my MTB cleats had similar problem once when the cleat moved and got stuck on the pedal (at least I laughed when I fell).

Mistakes happen to everyone, even the pros at the height of their careers, your ratios of time vs kms vs falling over is still probably amazing, as long as you didn't snap your wrist in the process it's all good!!

Interesting thing is there have been a few threads about worst wipeouts ever etc and the most common theme involved some form of falling over/over the bars whilst cleated...

In response to your question... yes... whilst you're on the floor

I have fallen soooo often that I have actually gotten good at it! Falling no longer bothers me.

Last week, was standing still, over my bike - chatting to a woman on the spruit while playing with her lab. I don't even clip in . Yet gravity managed to convince me to have a lie down.

I am GIFTED!!!! Maybe I should switch to needlework

My first and most spectacular fall was on a "trial" ride with a new club I wanted to join.  150m from the start and BOOM!  Never went back.  Those guys with their Toyota shirts were just too intimidating for a noob at the time!
