Pain in the Knee...

A friend of mine has been suffering with pain below his left patella while cycling, he has been for x-rays and physio. The Physio put him on rest and easy exercise for 3 month to try an rehabilitate the injury. After the 3 months off the bike we went for a very easy paced road ride on our mountain bikes on Saturday. All was going quite well until we ended up on a 1km stretch of downhill single track next to the road and the pain returned immediately to the point where he could hardly walk after that. It seems as if the section of rough track did the trick. 

 

Ok so originally his bike hadn't been set up 100% correctly, this was rectified.

The x-rays didn't show anything.

Apparently its ligaments.

 

Needless to say he is extremely pissed and worried at the same time, what I'd like to know is if any hubber has experienced similar and what the possible solutions are to rehabilitation so he can get back on his bike, clearly time and rest didn't help. Any suggestions would be welcome...

I know you said the bike setup was corrected but it still sounds that something is wrong.

It may be that your friend twists his knee when standing / riding something rough, this may be due to him "tensing" up on the rough stuff.

Maybe redo the cleat alignment which is often overlooked during bike setup.

I had the same problem. 

It ended up being the setup. I just moved my saddle backwards about 2 cm and up about 1 cm. The problem is much better now.

have to agree can be the cleats. if it "forces" the knee into an unnatural position it can hurt and do do damage as well.

Was the physio treating the symptom or the cause? i.e. pain relief through massaging/etc. vs rehabilitation via exercises/stretches. Is the pain only induced by cycling?

 

I've been rehabbing chronic knee pain induced by cycling/running for the last few months and it took weekly sessions with my physio/bio (and 1-2hr of daily exercising/stretching on my part) to track down the cause.

Rechecking bike setup is the easy answer then. But, if the pain is onset by anything other than cycling, that likely points to a more fundamental issue. For example, a skeletal joint structure preventing the knee from tracking correctly.

A physio will ease pain but not correct the cause of the pain. I would recommend he go and see a biokinetikist...and particular one that has some experience in cycling. I developed a pain in my knee about 4 weeks ago from running and have been seeing one to sort me out rather urgently before the end of the month. It has taken about 3 weeks of focused excercises and regular assessments of progress but I am now starting to see progress to the point where I am now happy to rest and let any inflammations die down and be relatively confident that it will not happen again. A good biokinitekist is well worth the investment

I changed from the Shimano 520 SPD to Time ATAC , as the Time pedal has 5 degrees of lateral movement . 

Seems to have sorted my problem out .
