Scuzzy please let us know if your friend has recieved help. I'm also kinda desperate. feeling very frustrated at the moment.

 

I was thinking of Jeroen Swart at the sports science institute, but the cost is very prohibitive

I had the same problem. Turned out i had a miniscus tear as well as osteo arthritis in my knees. The miniscus tear was cleaned up with a operation and the arthritis growths was scraped, also got a steroid injection to the knee area. Have had no pain what so ever since the opp was done today 3 weeks ago.

I ride with a knee brace. Problem reduced

 

What kind of brace are you using? My husband has got problems with ligaments in the back of his one knee and we are currently looking for some kind of knee brace to support his knee.

I used to have all kinds of pain on both knees. First it was an ITB like pain but was not right at the ITB. Went for an operation but the pain was still there. I tried deep tissue massage and it helps, only thing is its a bit costly. Then I also developed a pain on the top of my knee cap. Not deep. I then found a physio that told me to stretch. I now stretch all the time, before and after rides. Also use a roller to help "soften" the muscles while stretching. I recently did the karoo to coast and after 4.45hrs on bike had no pain. Knee pain of front of knee is without a doubt as result of too low saddle and tight muscles in my case.

would be keen to know as well. Unless it affects your day to day activities, there shouldn't be a reason.

Agree with tight muscles. Same thing happened with me, trained very hard for K2C, with all the cycling and no cross training the muscles in your legs gets to tight. need to go for sport massage once/twice a month.

OK so I have been diagnosed with a patellafemoral disorder of the knee. Has anyone experienced this? Did it turn out to be chronic and what course of action were or are you taking?

I was diagnosed with this at the end of last year. I've been to physios, bios and pretty much every bike fitter in Joburg. All of the recommendations have been for physio treatment to alleviate the symptons, bio exercises and setup to address muscle imblances and remove the probable cause of the problem. I haven't raced or done a hard ride since last November. I am slowly getting back to higher intensities, but my knee is still niggling and I worry that it will just come back as soon as I start racing again. I'm now working with another bike fitter.

 

I've spent a whole lot of money and am becoming seriously disillusioned about the whole thing.

Have you tried taping the patella? One of the causes is the patella not tracking properly, often due to muscle imbalance as said earlier. I've known taping to be very effective for some people. Also in the slightly longer term, supplementing with Glucosamine Chondroitin has also helped.

Hi,

 

Taping is something I'm looking at. I've started Glucosamine and MSM supplementation about 3 weeks ago. Benefits of Glucosamine supplementaion I'm told can only be felt longer term though.

This is my exact worry. Looks like a slippery road we on. I've read threads on the web where guys have been off their bike for a year to give their knee abreak, only for the problem to return the minute they get back on.

 

Something to consider as you are getting desperate and have probably tried all other options:

What cleats do you use and how much float do they have?

More float usually allows your knee more freedom to position itself naturally and relieving pressure on the knee.

 

Saddle position, try move the saddle back some more (only a few mm’s at a time) than what a bike fit person recommends.

Moving the saddle back can reduce pressure on the knee (note saddle height will also need to be adjusted lower when moving the saddle back, less than the amount you move back)

 

You feel the pain in your knee but the cause might be further up?

Are you sitting square or asymmetrically on the saddle, does one hip drop significantly more than the other?

If there is a significant hip drop on one side it could cause the other side knee to move in an abnormal fashion (outward movement).

Tilting the saddle nose slightly left or right a few mm’s can help to relieve a dropped hip scenario albeit more a temporary fix.

 

Stretch and massage regular, you need to try and keep the quad, hip flexor and hamstrings loose and supple.

Tight quad muscles on one side could be acting on your kneecap i.e. pulling it out of alignment.
