There are 50 different stories to be told if any one picture is chosen. Only the multiple video angles in entirety tell us the story, and even then, there's stuff we're oblivious to. But I saw that elbow more of a way to push off an already falling rider than anything else. In one of the (many) slo-mo replays it looks very much like Sagan's foot/pedal touched Cav's front wheel and caused the fall.
