I was warming up to meet my training group this morning and was inside the yellow shoulder on the way to Cape Town stadium. Then the TV channel switched and I was flung somewhere and, getting up, I couldn’t see the car. The rear view mirror and fender guard left behind on the scene was off a Renault Kwid, not many of them around.
I am privileged to have an excellent health care network around me and hope to fully recover - very eina right leg, fractured ribs and scrapes and bruises, fortunately my spine and brain intact.

I had an emergency Laminectomy last week Tuesday after a bulging lumbar disc almost completely squashed my Sciatic nerve. (whilst doing my morning ablutions and getting comfy on the throne before my ride😪...)
So I'm basically bedridden for three weeks, then rehab and only after about three months can I slowly get going on the trainer again.
But I must admit, the discomfort from the operation wound is a LOT better than that searing pain running from my glute all the way down to my calf. It was hell, I did not know how to sit, stand or lay down without the most excruciating pain.
But fear not, I'll be back for no. 20 next year......
Been through this procedure 3 bloody times (1994, 2008 & 2013), fortunately life can be back to normal after 6-8 weeks……. just do the recovery rehab exercise sessions!

My take on this is the required commitment to a change in lifestyle, changing something to lose the weight and once the weight is lost reverting back to doing what you did will take your weight back to where you started.
I lost 16kgs over a 6 week period from 25 Nov 2019 (102kg's) to 3 Jan 2020 (86kg's) participating in a carb cycling diet (controlled portions of healthy whole foods) and boot camp style exercise (5 x 40min sessions a week). No sugar, alcohol, fruits or "colorful" veggies. Every 4th day carbs could be consumed in controlled portions. Eating like this took some planning for meal prep (it is not sustainable, not for me at least), but it did what I wanted it to do, which is kick start the healthier lifestyle.
Now 2 years down the line, I way 81kg's (plus or minus a 1kg), watching what I eat - not anal about it - combined with a mix of walking, running, cycling and gym exercises.
In general, life is much better, feeling healthy and strong, no colds or flu over the past 2 years including Covid!
I stand under correction, but do not believe it has to be a OneUp stem, I have the edc tool fitted into an Industry 9 stem.
I endured the pain for 6 months, along with money wasted on physio and a chiropractor before going under the knife or eating more painkillers to upset some other body part. Had a good surgeon, pain relief after surgery was immediate, aside the new cut from the surgery.
Started cycling MTB late in 2013 after the 3rd op, since this pandemic started I have mainly been on the road bike.
Keeping your core strong and weight off the belly goes along way to ease the burden carried by your lower back!
Get the correct size bike and a fit too, all to help with posture.
I have had three procedures all being the laminectomy and discectomy - no spinal fusions. First one on 1994 between L5 and S1, second in 2008 between L4 and L5 (right side) and the third in 2013 also between L4 and L5 but on the left side. All three instances resulting in excruciation referred leg pain owing to the sciatic nerve being pinched. All three procedures where successful....I just have a generic disc degeneration, which extends up into my neck. Critical to follow the recovery procedure and exercises spelled out by the physio and pretty much do nothing strenuous for the first 6 weeks, limit the amount of sitting, so I would basically stand or lie down. To date, I cannot complain about life afterwards, but I do take general caution.

I suspect you may need to revisit your eating habits, it is a lifestyle change, which is rewarding once into the "flow" but does present its challenges, especially under the current up & down of lock down levels and its concomitant stresses..!
